Annual evaluation forms should be submitted in order to get your certificate.

Q: What should I do if I cannot access my annual assessment form?

A: Kindly ensure  you are part of the outgoing batch. Also, ensure that the enrollment status on your dashboard shows “User Agency Acceptance, Approved By Region”.

If it shows, “Personnel Posted”, it means you did not do the registration at the beginning of the service. You should print your appointment letter from your dashboard and have your company endorse it and then submit it at the regional office for validation. 

Q: How do I request a change of name, email or picture if I failed to do that before printing the evaluation form? 

A: Create a ticket with your index number and attach a valid national ID and the new email. For picture change, attach an appropriate picture as attachment to the ticket.
